Checklist for doing your test BY POST:

• Have you completed section A and section B?
• Have you stuck the green label on the urine pot?
• Did you take your sample from the first part of the flow?
• Have you torn off and kept the white cover sheet with the screening office phone number on it?
• Have you put your sample bottle in the moulded plastic container and closed it firmly?
• Have you put the form and sample bottle in the green-and-white striped plastic envelope and sealed it?

YOU SHOULD SEND BACK YOUR URINE SAMPLE ON THE SAME DAY THAT YOU DO IT.

NO STAMP IS REQUIRED ON THE ENVELOPE.
